Hartford Health Initiative (HHI) was founded in 2017 by its Founding Executive Director, Chavon Hamilton-Burgess, with the sole mission of improving health outcomes for residents in North Hartford (Blue Hills, Clay Arsenal, Northeast, and Upper Albany) through education, community initiatives, and programming. Our work is rooted in community-based research methods, using lived experiences to guide the development of our response. Since our inception, we have provided residents across the lifespan with essential resources, knowledge, and access to care. At HHI, we believe that “Your Zip Code Should Not Dictate Your Health”.
Chavon Hamilton-Burgess, MSW, MPA, is a lifelong resident of Hartford, CT, the community that HHI serves. Her purpose and passion drive the mission and vision of the work of the organization. Chavon believes that health is a human right and intertwined in the fabric of our daily lives. If individuals do not have the ability to be the healthiest they can be, they are unable to actualize their maximum potential.
Chavon’s interest in public health started in 2006. Prior to founding HHI, Chavon conducted public health research in Hartford, CT at the Institute for Community Research and subsequently at Emory University in Atlanta, GA. Before returning to Hartford, she led the Chlamydia efforts, decreasing rates of transmission and contraction among Black and Latino/a youth ages 15-24, in the City of Boston.
Chavon holds a Bachelor of Science Degree of University of Hartford, a Master of Social Work degree from the UCONN School of Social Work and a Master of Public Administration degree from the Rutgers University School of Public Affairs and Administration. She is currently a PhD Candidate at UMASS Amherst School of Public Health and Health Sciences.
Fatima Martinez has been with HHI for two years, first as an intern and subsequently leading our care coordination program. She is passionate about health equity and community wellness, helping individuals access quality healthcare, navigate complex systems, and connect with essential community resources. She works collaboratively with clients, healthcare providers, and community partners to deliver compassionate, person-centered care that improves health outcomes. Through advocacy, collaboration, and coordinated care, Fatima is committed to empowering individuals and families with the support and services they need to thrive.
Fatima earned her Bachelor of Arts in Public Health in 2025 and holds a Certificate in Integrated Health.
